To register a death, you must bring a Death Notification Form stating the cause of death to the Registrar of Births, Marriages and Deaths most convenient to you. Deaths should be registered as soon as possible and no later than 3 months from the date of the death.

Once it has been verbally confirmed that the doctor is in a position to issue the Death Notification Form, arrangements for the funeral may proceed.

Death Notification Form

When a death takes place at home, in a hospital, hospice, nursing home or other institution, a registered , medical practitioner who attended the deceased  completes Part 1 of the Death Notification Form (Medical Certificate for the Cause of Death).

Part 2 of the form provides additional personal details of the deceased and is completed by the deceased’s civil partner or a close family relative, acting as a Qualified Informant. If such a person is not available, the registrar has a hierarchy of people who can act as a Qualified Informant.

If the deceased doctor did not see the deceased at least 28 days before the death occurred or if the doctor is unhappy about the cause of death, he/she must inform a Coroner who will decide if a post-mortem is necessary.
All sudden deaths, accidents etc., are by law subject to a Coroner Post Mortem.
In the case of a post mortem taking place, the Coroner issues a Coroner’s Certificate of the cause of death to the Registrar of Births, Marriages and Deaths and the death is automatically registered.

Removals can take place from Tuohy Funeral Directors Funeral Home, your Residence or from Hospital.

Tuohy Funeral Directors’  Funeral Homes are especially designed to meet your family’s needs. We can facilitate your removal being open to the public or kept private – family only.

If you choose to have a traditional Wake at home, we will help you in every way we can. We will bring the deceased to our funeral home to be prepared for burial and then return the deceased to your residence, until it is time for removal to the Funeral Ceremony.

Most Hospitals would have a mortuary for funeral use. We can work with the Hospital on your behalf to arrange a removal ceremony in the hospital mortuary if that is what you want.
Most hospitals now insist on embalming. This would have to be done in our Funeral Home and then the body would be returned to the hospital mortuary.

A Death Notice normally appears in a newspaper, local radio and RIP.ie. We at Tuohy Funeral Directors understand how difficult it is to write the Death Notice for a loved one. And the importance of not forgetting to name a family member or close friend of the deceased.

We will work with you to write and forward a death notice to your chosen newspaper and radio station. We will also work with you to add the death notice to RIP.ie. A death notice may be added, and amended, at any time free of charge on RIP.ie.

Tuohy Funeral Directors will work with you in arranging the Funeral Ceremony that is appropriate to the wishes of the deceased and the needs of you and your family.

We can help you organise a religious service. This normally takes place in the parish church of the deceased but it can take place in a church or other place of worship of your choosing.

Alternatively, if you prefer a non-religious service, we will work with you to organise a civil ceremony to be held in our funeral home or another location of your choice.

If a family plot exists it should be used, and there is space in the grave for the burial, but you will need to show ownership of the burial plot.
If a new plot is required, we can advise you on the plots that are available to you.

The burial fees charged by cemeteries usually range from €700 to €1200, and are charged whenever a burial takes place. The cost of purchasing a new grave ranges from €1200 to €5000, although, in some cemeteries, new graves can cost a lot more.

If you decide on cremation, we will provide a cremation service tailored to your needs. We will also complete all the necessary forms and documents on your behalf, ensuring the service is as stress-free as possible.

We can organise a full funeral service with the Funeral Mass in your local church before we transfer your deceased to the crematorium of your choice, for a short prayer service in the crematorium’s chapel and cremation.

Alternatively we can bring your deceased from hospital/nursing home/ home directly to the chapel of the crematorium for the main ceremony and cremation.

We will also help you choose the urn or casket that is right for you. Families tend to choose an urn for scattering, a wooden casket for burial in family grave or a more ornate urn if the ashes are to be kept at home for a lengthy period.

Ashes are usually available two to three days after the cremation service.
